What to Expect

If you are applying for adjustment of status to become a lawful permanent resident (Green Card), United States Citizenship and Immigration Services (USCIS) requires a comprehensive medical examination completed by a designated Civil Surgeon.

At Monroeville Direct Primary Care, Dr. Johnson provides unhurried, transparently priced, and respectful care. Dr. Kristina Johnson is a board-certified family physician who ensures your paperwork is completed accurately and efficiently to avoid unnecessary delays.

How it Works

  1. Initial Visit: Dr. Johnson reviews your medical history, vaccination records, and performs the required physical examination.

  2. Lab Testing: You will visit a local Labcorp location for your required blood and urine collections.

  3. Return Visit: Once results are back, you return to our office to review the findings and pick up your certified, sealed Form I-693 envelope.

Important Notes for Patients:

Potential Additional Lab Fees: If a screening test returns an abnormal result, Labcorp will automatically run a mandatory confirmatory test, which incurs an additional charge ($9.49 for syphilis).

Age & Vaccine Requirements: Depending on your age and documented vaccine history, Dr. Johnson may need to order additional immunity titers or send you for a chest X-ray. These are priced transparently as add-ons if needed.

Important Notes for Patients:

  • Payment for Chest X-Rays: The $75.00 fee for a chest X-ray is subject to change and is paid directly to the Monroeville Imaging Center
  • Potential Additional Lab Fees: If the HIV screening test returns an abnormal result, Labcorp will automatically run a confrmation test, which incurs an additional charge of up to $226.79
  • Why get a titer? If you do not have official, translated records of your vaccinations, a titer blood test can prove your immunity to USCIS so you can avoid paying for and receiving the vaccines again.

What About Missing Vaccinations?

USCIS requires applicants to be up-to-date on age-appropriate immunizations. If our evaluation or your titer results show you are missing a required vaccine, you have options:

  1. Get them at a pharmacy: You can take Dr. Johnson’s order to a local pharmacy to see if they are covered by your health insurance.

  2. Allegheny County Health Department: You can obtain common vaccines directly through the health department.

For your planning purposes, the health department's standard self-pay pricing is available on their website.

What to Bring to Your Appointment

To ensure a smooth visit, please bring the following items your appointment:

  • Valid Government-Issued Photo ID (such as a valid passport)

  • All Prior Immunization Records (translated into English, if applicable)

  • A Printed Copy of Form I-693 with your personal information filled out in black ink (do not sign the form until instructed by Dr. Johnson)

  • A comprehensive list of your current medications and medical history
