What to Expect

If you are applying for adjustment of status to become a lawful permanent resident (Green Card), United States Citizenship and Immigration Services (USCIS) requires a comprehensive medical examination completed by a designated Civil Surgeon.

At Monroeville Direct Primary Care, Dr. Johnson provides unhurried, transparently priced, and respectful care. Dr. Kristina Johnson is a board-certified family physician who ensures your paperwork is completed accurately and efficiently to avoid unnecessary delays.

How it Works

  1. Initial Visit: Dr. Johnson reviews your medical history, vaccination records, and performs the required physical examination.

  2. Testing and Vaccines: You will visit a local Labcorp location for your required blood and urine collections. If needed, Xrays will be performed through Monroeville Imaging Center. Vaccines can be obtained through the Allegheny County Health Department or a local pharmacy.

  3. Return Visit: Once results are back, you return to Dr. Johnson’s office to review the findings and pick up your certified, sealed Form I-693 envelope.

What to Bring to Your Appointment

To ensure a smooth visit, please bring the following items your appointment:

  • Valid Government-Issued Photo ID (such as a valid passport)

  • All Prior Immunization Records (translated into English, if applicable)

  • A Printed Copy of Form I-693 with your personal information filled out in black ink (do not sign the form until instructed by Dr. Johnson)

  • A comprehensive list of your current medications and medical history
